Bigger Picture

Andrew S. Gurwood, O.D.

History: A 31-year-old, black female presented for a second opinion regarding the treatment of uveitis in her right eye. The patient had been taking cyclopentolate 1% qd and prednisolone acetate 1% qid for the past year.

She reported no other pertinent ocular history. Her family history was positive for glaucoma. Her systemic history was unremarkable, and she denied any drug allergies.

This 31-year-old black female presented with these biomicroscopic and dilated fundus exam findings (O.D. top, O.S. bottom).

Diagnostic Data: This patient’s best-corrected visual acuities were 20/40 O.D. and 20/60 O.S. at distance and near. External examination was unremarkable, and there was no afferent pupillary defect. Refraction revealed compound myopic astigmatism with no improvements.

Goldmann applanation intraocular pressures measured 32mm Hg O.D. and 34mm Hg O.S. Gonioscopy with a 4-mirror lens demonstrated open angles, flat iris approach and grade II TM pigment O.U. The biomicroscopic and dilated fundus examination findings are seen in the photographs.
