Appearance and Pain: More than
Meets the Eye

History: A 65-year-old black female complained of a painful,
left eye with crusty discharge on the eyelids for 2 weeks. Her ocular history
was otherwise unremarkable. Her systemic history was notable for hypertension
for the past 17 years. She also complained of a chronic, untreated “cold”
for the past 3 weeks.
Diagnostic Data: Best-corrected visual acuity was 20/25 O.U.
at distance and near. The external examination was normal with no evidence
of afferent pupillary defect. Refraction revealed myopia with astigmatism
and presbyopia O.U.

This photograph illustrates the slit lamp findings. The affected region
was warm to the touch and sensitive. IOP was 16mm Hg O.U., and both angles
were open. The dilated fundus exam was normal.
