Appearance and Pain: More than Meets the Eye

Andrew S. Gurwood, O.D.

History: A 65-year-old black female complained of a painful, left eye with crusty discharge on the eyelids for 2 weeks. Her ocular history was otherwise unremarkable. Her systemic history was notable for hypertension for the past 17 years. She also complained of a chronic, untreated “cold” for the past 3 weeks.

Diagnostic Data: Best-corrected visual acuity was 20/25 O.U. at distance and near. The external examination was normal with no evidence of afferent pupillary defect. Refraction revealed myopia with astigmatism and presbyopia O.U.
